A lot of people who would benefit from acupuncture put it off simply because they don’t know what to expect. Will it hurt? How many needles? How long will it take? Will it work the first time?

These are questions we hear every day at Rishaan Physio & Wellness Clinic in Mississauga. Here’s exactly what happens at your first appointment — so you can walk in knowing what to expect.

Before You Arrive

  • Eat something beforehand — don’t come on an empty stomach. A light meal or snack an hour or two before your appointment is ideal. Acupuncture relaxes the body deeply, and people who haven’t eaten can feel lightheaded.
  • Wear comfortable, loose clothing — needles are often placed on the lower legs, feet, and forearms, so clothing you can easily roll up makes things simpler. You won’t need to fully undress.
  • Skip the tongue scraping that morning — your acupuncturist may look at your tongue as part of the assessment, and the natural coating is part of what they need to see.
  • Avoid alcohol the same day — it can interfere with how your body responds to treatment.

When You Arrive: The Intake

Your first appointment is longer than follow-up sessions — usually 60 to 75 minutes — because it includes a thorough health history before treatment begins.

Your acupuncturist will ask about what brings you in, how long you’ve had the issue, what makes it better or worse, and your overall health — including your sleep, digestion, energy, stress, and mood. These questions might feel broader than expected, but they’re all relevant. Acupuncture treats the whole person, so understanding your overall health picture is part of getting the treatment right.

They may also check your pulse on both wrists and look at your tongue — a standard part of Traditional Chinese Medicine assessment.

The Needles: What You Actually Feel

This is the question everyone asks first — and the honest answer is that it’s much less uncomfortable than most people expect.

Acupuncture needles are extremely fine — about the thickness of a human hair — and nothing like the needles used for injections. Insertion is typically felt as a brief, mild prick that lasts less than a second. Many points barely register at all.

Once the needle is in place, you might feel a mild sensation — a slight heaviness, warmth, or dull pressure at the site. This is normal and usually fades within a minute or two. If anything feels sharp, stinging, or uncomfortable, just tell your acupuncturist — they can easily adjust.

All needles at Rishaan Physio are sterile and single-use. There is no reuse of needles at any time.

During the Session

After the needles are placed — usually 8 to 15 of them, depending on what’s being treated — you’ll rest on the treatment table for about 20 to 30 minutes while they do their work.

This is the part most first-timers are surprised by. It’s deeply relaxing. The room is quiet, you have nothing to do, and the treatment is working in the background. Many patients fall asleep, which is completely normal — it actually means the relaxation response is working well. Your acupuncturist will check in during this time and let you know when it’s done.

Removing the needles is even less noticeable than placing them — they slide out easily, and most people barely feel it.

How You’ll Feel Afterwards

Most people feel calm and relaxed after their first session — a pleasant heaviness, a sense of stillness. Some feel energised; others feel a bit tired. Both are normal responses, and both usually pass within a few hours.

Occasionally, a first session can be followed by a brief day of feeling a bit “off” before things improve — this is more likely with chronic conditions and usually resolves within 24 to 48 hours.

After Your Session

  • Take it easy for the rest of the day — avoid intense exercise or stressful activities; your body benefits from some rest after treatment
  • Drink water — staying hydrated supports the treatment
  • Notice what changes — even small shifts in sleep, pain, energy, or mood are worth noting; your acupuncturist will ask at your next session

How Many Sessions Will You Need?

It depends on your condition. Acute issues may respond in two to four sessions. Chronic conditions that have been present for a while typically benefit from a course of 6 to 10 weekly sessions before consistent improvement settles in. Your acupuncturist will give you a clear recommendation at the end of your first visit.

Many first-time patients notice something after even their first session — better sleep that night, reduced pain, or just feeling more relaxed and settled than they have in a while.
