  • Perform ankle pumps, quad sets, and heel slides several times a day
  • Work toward getting the knee fully straight (this is critical and gets harder the longer it is delayed)
  • Begin bending the knee, aiming for roughly 70–90 degrees by the end of the week
  • Walk short distances indoors with your aid
  • Ice and elevate the leg regularly

Weeks 2–3: Building Range of Motion

Goals: full or near-full extension, 90–100 degrees of bend, walking with less support.

Swelling is still present but decreasing. Physiotherapy sessions now include hands-on mobilization of the kneecap and joint, assisted bending and straightening, and progressing your exercises. Many patients transition from a walker to crutches or a cane during this period. Staples or stitches are typically removed around the two-week mark.

Weeks 4–6: Strength and Independence

Goals: 110–120 degrees of bend, walking without an aid, stairs with confidence.

With movement largely restored, the focus shifts to strengthening. Sessions include:

  • Stationary cycling — usually possible once the knee bends past 100 degrees
  • Mini squats, step-ups, and leg press to rebuild the quadriceps
  • Balance and proprioception exercises
  • Gait retraining to eliminate limping — a limp that persists becomes a habit and stresses the hip and back
  • Stair practice, both up and down

Most patients return to driving around four to six weeks (right knee) or sooner (left knee with an automatic car) once cleared by the surgeon and able to brake safely.

Weeks 6–12: Return to Daily Life

Goals: 120+ degrees of bend, walking longer distances, return to work and light recreation.

Strengthening intensifies and becomes more functional — getting up from low chairs, kneeling (if the surgeon allows), carrying groceries, walking on uneven ground. Endurance improves week over week. Many patients return to desk work at six to eight weeks; physically demanding jobs take longer. Low-impact activities such as swimming, cycling, and walking programs begin here.

Some swelling and warmth in the knee is still normal at this stage and can persist for several months.

Months 3–12: Long-Term Recovery

Formal physiotherapy typically winds down between three and four months, but recovery continues for up to a year. By this stage you should be walking comfortably, managing stairs normally, and returning to activities like golf, cycling, gardening, and travel. Your physiotherapist will give you a maintenance program to continue building strength and keep the knee moving well for years to come.

High-impact activities such as running and jumping are generally not recommended for replaced knees, but the majority of patients return to an active, pain-free lifestyle.
